Fresh whole blood from IRB-consented donors with clinically confirmed ankylosing spondylitis, an immune-mediated inflammatory arthritis of the spine and sacroiliac joints strongly associated with HLA-B27 and driven by the IL-17 axis rather than by autoantibodies. Whole blood supplies genomic DNA for HLA-B27 and ERAP1 genotyping and retains granulocytes for transcriptomic and ex vivo stimulation work that PBMC preparations discard. Donors can be stratified by HLA-B27 status, BASDAI or ASDAS activity and treatment class.
Ankylosing Spondylitis Whole Blood for Spondyloarthritis and HLA-B27 Research
Ankylosing spondylitis is an immune-mediated inflammatory arthritis primarily affecting the spine and sacroiliac joints, characterised by inflammation at entheses followed by new bone formation, syndesmophytes and eventual ankylosis. It is strongly associated with HLA-B27 and is a seronegative disease — there is no diagnostic autoantibody, which distinguishes it mechanistically from rheumatoid arthritis and other classically autoimmune arthritides. Sanguine collects fresh whole blood from IRB-consented donors with a clinically confirmed ankylosing spondylitis diagnosis for rheumatology, immunology and genetic research.
Why Whole Blood for Ankylosing Spondylitis Research
- HLA-B27 is by far the strongest known genetic risk factor for ankylosing spondylitis, carried by the great majority of patients. Genotyping and HLA typing require genomic DNA, and whole blood is the direct source.
- Whole blood also supplies DNA for the other established risk loci, notably ERAP1, whose gene-gene interaction with HLA-B27 is one of the better-characterised epistatic relationships in complex disease genetics.
- Whole blood transcriptomics retains neutrophils and other granulocytes, which density-gradient PBMC preparations discard. This matters in a disease where IL-17-driven neutrophil biology is central to the inflammatory programme.
- Ex vivo whole blood stimulation assays preserve physiological cell composition together with the donor’s own plasma proteins, giving a more representative cytokine release readout than isolated cells in defined media.
- The disease responds to TNF and IL-17A blockade, while IL-23 blockade has not shown efficacy in axial disease despite the IL-23/IL-17 pathway being implicated — a divergence from psoriasis that makes patient-derived cellular material valuable for mechanistic work.
- Whole blood permits flow cytometric immunophenotyping without the selection bias introduced by density-gradient separation, and CRP and inflammatory markers can be assessed from the same draw.
