About Anti-MAG Neuropathy
Anti-MAG neuropathy is an autoimmune demyelinating peripheral neuropathy driven by IgM autoantibodies against myelin-associated glycoprotein, a component of the periaxonal Schwann cell membrane. In most cases the IgM is monoclonal and arises from an underlying IgM monoclonal gammopathy of undetermined significance (MGUS) or, less often, Waldenström macroglobulinaemia — so the condition sits at the intersection of neurology and clonal B-cell disease.
Clinically it presents as a slowly progressive, distal, predominantly sensory neuropathy with prominent sensory ataxia and tremor — the phenotype often described as DADS (distal acquired demyelinating symmetric neuropathy). Because pathogenicity correlates with anti-MAG antibody titre, and because the antibodies are the direct effector, donor serum is the primary research material rather than a surrogate: it contains the pathogenic species itself.
