Why Lesional Tissue Matters in Psoriasis

Psoriasis is a chronic immune-mediated skin disease driven by the IL-23/Th17 axis, and the pathology is visible in tissue in a way it is not in blood. A plaque biopsy shows the defining histology directly: epidermal hyperproliferation with acanthosis, parakeratosis and loss of the granular layer, elongated rete ridges, dilated dermal capillaries, and a mixed infiltrate of T cells, dendritic cells and neutrophils — including the Munro microabscesses characteristic of the disease.

That makes skin punch biopsies the primary material for target validation in psoriasis, particularly for the IL-17 and IL-23 pathways underlying current biologics. Because uninvolved skin from the same donor is genetically identical but phenotypically normal, paired lesional and non-lesional sampling is the strongest available design — it controls for donor genetics entirely and isolates the disease signal.
