Alopecia Areata Skin Punch Biopsy samples are collected from IRB-consented donors with a clinically confirmed diagnosis of alopecia areata by licensed clinicians under aseptic conditions. Alopecia areata is an autoimmune disease of the hair follicle: the immune privilege that normally protects the anagen hair bulb collapses, MHC class I expression rises in follicular epithelium, and CD8+ T cells attack the follicle, producing non-scarring hair loss. Skin punch biopsy is the only specimen that shows this lesion in situ.

Why Skin Punch Biopsy for Alopecia Areata Research

  • The defining histological finding is a dense peribulbar lymphocytic infiltrate around anagen follicles — the classic “swarm of bees” appearance. Its spatial relationship to the follicular bulb is the lesion itself and cannot be reconstructed from blood.
  • Horizontal sectioning of scalp punch biopsies permits quantitative follicular counts, terminal-to-miniaturised follicle ratios and anagen-to-telogen ratios, which are the standard morphometric endpoints in hair research.
  • Tissue distinguishes alopecia areata from its principal differentials in a way clinical assessment often cannot: scarring alopecias destroy the follicle and replace it with fibrous tracts, while androgenetic alopecia shows miniaturisation without a peribulbar infiltrate.
  • Interferon-gamma signalling through the JAK-STAT pathway is central to immune privilege collapse, and the JAK inhibitor class approved for severe alopecia areata acts on exactly this axis. In situ assessment of pathway activation and of the IFN-inducible chemokines CXCL9, CXCL10 and CXCL11 requires tissue.
  • MHC class I and immune privilege marker immunostaining of the follicular epithelium is the direct readout of the mechanism that permits autoimmune attack.
  • Lesional and non-lesional or unaffected scalp biopsies from the same donor provide an internal comparator for spatial transcriptomic and immunohistochemical work.
