Crohn’s disease is an immune-mediated inflammatory bowel disease characterised by patchy, transmural inflammation that can affect any segment of the gastrointestinal tract. Whole blood retains leukocytes, platelets, erythrocytes, and the full plasma compartment, making it the most flexible starting material for transcriptomic, genomic, and immune-cell work.
Crohn’s Disease Biology and Why Whole Blood
- Crohn’s disease is immune-mediated rather than autoantibody-driven: susceptibility loci including NOD2, ATG16L1, IRGM, and IL23R implicate innate bacterial sensing, autophagy, and IL-23 signalling in defective handling of the gut microbiota.
- The IL-23/Th17 axis together with TNF and JAK-STAT signalling underpins the effector response, and these pathways are the targets of the approved biologic and small-molecule classes used in the disease.
- Transmural inflammation with non-caseating granulomas, fistulising and stricturing behaviour distinguishes Crohn’s disease from ulcerative colitis and drives distinct clinical phenotypes.

- Treatment exposure: biologic-naive, anti-TNF, anti-integrin, anti-IL-12/23 or anti-IL-23, or JAK inhibitor treated
