Cytomegalovirus is a ubiquitous human betaherpesvirus that establishes lifelong latency after primary infection, with periodic reactivation controlled by the host immune response. Host Immune Response to CMV Infection and Why Serum

• Primary CMV infection elicits a virus-specific antibody response; IgM typically marks recent or reactivated infection while IgG persists indefinitely and defines serostatus. Both are serum analytes, which makes serum the standard matrix for CMV serological work.
• IgG avidity maturation over time after primary infection is used to help distinguish recent from remote infection, and avidity assays are performed on serum.
• After primary infection CMV persists latently, and control of reactivation depends on cellular immunity — a major reason CMV serostatus is a critical variable in transplantation and immunosuppression research.
• CMV drives a strong and durable adaptive immune response and is widely used as a model antigen system in human immunology, including immunosenescence and inflammaging research.
