Disease Immunology and Why Whole Blood

  • SLE is a multi-system autoimmune disease characterized by loss of tolerance to nuclear antigens, with antinuclear antibodies and anti-double-stranded DNA antibodies as defining serology; anti-Sm and anti-Ro/SSA are also common.
  • Nucleic acid-containing immune complexes are internalized and signal through endosomal nucleic acid sensors, driving type I interferon production. The resulting interferon transcriptional signature is one of the most reproducible molecular features of the disease and is the basis for interferon receptor-directed therapy.
  • B cell survival factor signaling is a validated therapeutic axis, and complement consumption with low C3 and C4 accompanies active disease, so both cellular and soluble readouts are informative.
  • Neutrophil biology contributes as well, including release of neutrophil extracellular traps that expose nuclear antigen. Neutrophils are lost when PBMCs are isolated, which is a specific argument for whole blood.
  • Whole blood preserves all leukocyte populations plus plasma in a single specimen, supporting whole-blood transcriptomics, intracellular signaling and phospho-flow work, and DNA extraction from the same donor draw.
  • SLE is clinically heterogeneous, ranging from predominantly cutaneous and articular disease to severe renal involvement, so organ-annotated donors are essential for interpretable results.
